Three-dimensional Finite Element Analysis on Effects of Three Kinds of Fiber Annulus Treatment Methods on Lumbar Spine Biomechanics

Abstract:
Objective To evaluate the effects of three fiber annulus treatments, including horizontal shape incision, annular incision and ?ber annulus suture, on lumbar spine biomechanics by establishing a ?nite element analysis model. Methods The DICOM results of CT of healthy adult L4/ 5 lumbar segments were selected. The normal lumbar vertebrae model, annular incision model, horizontal incision model and suture model were established by reverse modeling. Biomechanical data of lumbar spine under normal axial pressure were measured under four conditions: ?exion, extension, lateral bending and rotation. Results Stress data of normal vertebral body and three different annulus fibrosus models were calculated by finite element method. It could be seen that the maximum stress of the annulus ?brosus and nucleus pulposus in annulus incision models under different conditions were higher than that of the normal model. After the ?ber annulus was sutured, the maximum stress under the ?exion and extension conditions was lower than the horizontal shape incision and the annular incision. Compared with the unsutured model, the different parts of the annulus ?brosus suture model were more average. Conclusion After sutured the annulus fibrosus, the stress distribution of the intervertebral disc, nucleus pulposus and annulus ?brosis is more average than the unsutured models, and the stress of the annulus caused by annulus incision is alleviated, which can positively maintain the stability of the intervertebral disc and prevent disc herniation recurrence.
