针刺强刺激疗法联合西医疗法对泌尿系结石肾绞痛的治疗效果

摘要:
目的:探讨针刺强刺激疗法联合西医疗法对急诊泌尿系结石肾绞痛的疗效。方法:选取2023年8月—2024年9月在石家庄市中医院急诊就诊的60例泌尿系结石肾绞痛患者,按随机数字表法随机分为常规组与针刺组,每组各30例。常规组采用常规解痉止痛治疗,针刺组在此基础上联合针刺强刺激疗法。观察并比较两组治疗前后临床症状变化、疼痛持续时间及补救镇痛发生率,同时检测治疗前后血清5-羟色胺(5-HT)、P物质(SP)及肾损伤分子1(KIM-1)水平。结果:两组治疗后10 min和治疗后30 min的疼痛数字评价量表(NRS)评分均低于治疗前,针刺组评分更低,且随着时间变化,表现出显著的时间×组间交互作用(P <0.05)。治疗后,两组患者的Wong-Baker面部表情量表和McGill疼痛量表(SF-MPQ)评分较治疗前降低,且针刺组患者评分均低于常规组,差异有统计学意义(P <0.05)。针刺组患者的疼痛持续时间短于常规组,差异有统计学意义(P <0.05)。治疗后,两组的5-HT和SP较治疗前降低,KIM-1水平较治疗前升高,且针刺组5-HT、SP和KIM-1水平均低于常规组,差异有统计学意义(P <0.05)。针刺组不良事件发生率为6.67%,常规组为20.00%,差异无统计学意义(P >0.05)。结论:针刺强刺激疗法联合常规西医治疗能有效缓解急诊泌尿系结石肾绞痛患者的疼痛症状,缩短疼痛持续时间,降低机体应激反应,且不良事件较少,值得临床应用推广。

Therapeutic effect of acupuncture and moxibustion combined with conventional western medicine in patients with renal colic and urinary calculi

Abstract:
Objective To analysis of the therapeutic effect of acupuncture and moxibustion combined with conventional western medicine in emergency patients with renal colic and urinary calculi. Methods A total of 60 emergency patients with renal colic and urinary calculi treated in Shijiazhuang Hospital of Traditional Chinese Medicine were enrolled between August 2023 and September 2024. According to random number table method, they were divided into routine group (n=30, routine relieving spasm and pain) and acupuncture group (n=30, acupuncture strong stimulation therapy on basis of routine group). The changes of clinical symptoms before and after treatment, pain duration and incidence of remedial analgesia were compared between the two groups. The levels of serum 5-hydroxytryptamine (5-HT), P substance (SP) and kidney injury molecule 1 (KIM-1) were detected before and after treatment. Results At 10 min and 30 min after treatment, scores of numerical rating scale (NRS) were decreased in both groups, which were lower in acupuncture group, and there was significant time × inter-group interaction with time (P <0.05). After treatment, scores of Wong-Baker facial expression scale and Short-form McGill Pain Questionnaire (SF-MPQ) were decreased in both groups, which were lower in acupuncture group (P <0.05). The pain duration in acupuncture group was shorter than that in routine group (P <0.05). After treatment, levels of 5-HT and SP were decreased, while KIM-1 was increased in both groups, and levels of 5-HT, SP and KIM-1 were lower in acupuncture group (P <0.05). There was no significant difference in the incidence of adverse events between acupuncture group and routine group (6.67% vs 20.00%, P >0.05). Conclusion Acupuncture strong stimulation therapy combined with routine western medicine provides more effective pain relief, shortens pain duration, and reduces stress responses in emergency patients with renal colic due to urinary calculi, with a favorable safety profile. It is worthy of clinical application.
