Influence of modified Kushen decoction sitz bath combined with catheter irrigation and drainage on wound healing, wound secretion and oxidative stress in patients with perianal abscess

Abstract:
Objective To explore the effect of modified Kushen decoction sitz bath combined with catheter irrigation and drainage on wound healing, wound secretion regulation and oxidative stress in patients with perianal abscess. Methods In this study, 320 patients with perianal abscess who received surgical treatment in the hospital were selected from February 2021 to February 2024 as analysis samples, and were randomly divided into two groups: control group and study group, with 160 cases in each group. The patients in the control group were treated with povidone iodine sitz bath after abscess incision and thread-drawing catheter irrigation and drainage, while the patients in the study group were combined with modified Kushen decoction sitz bath on the basis of the control group. Both groups were treated for 14 days. The changes in TCM syndromes scores, wound healing status [anal visual analogue scale (VAS) score, granulation growth score, wound area], wound secretion conditions (wound secretion score, exudate secretion disappearance time) and oxidative stress [superoxide dismutase (SOD), malondialdehyde (MDA)] were compared before and after treatment, and the occurrence of adverse reactions in the two groups during treatment was observed. Results After 14 days of treatment, the scores of anal bulge, anal wet itching, anal sinus congestion, tenesmus and dry mouth constipation and level of MDA in the two groups were significantly decreased while the SOD level was significantly increased, and the scores and MDA in study group were lower than those in control group while the level of SOD was higher (P <0.05). After 7 days of treatment and 14 days of treatment, the anal VAS, granulation growth status, wound secretion score and wound area in both groups were significantly reduced than those after 1 day of treatment, and the above indicators in the two groups were significantly declined after 14 days of treatment compared with those after 7 days of treatment (P <0.05). After 7 days and 14 days of treatment, the anal VAS, granulation growth status, wound secretion score and wound area were significantly lower in study group than those in control group (P <0.05). The disappearance time of exudate secretion in study group was significantly shorter than that in control group (P <0.05). During treatment, there were no obvious adverse reactions in study group and control group (P >0.05). Conclusion Comprehensive treatment with modified Kushen decoction sitz bath and catheter irrigation and drainage has a significant effect on alleviating the clinical symptoms of patients with perianal abscess. This therapy can promote wound healing, reduce wound secretion, relieve anal pain and correct oxidative stress status.
