Impacts of Huayu Sanjie decoction and microwave on pain mediators and serum levels of chemokine in patients with ring mixed hemorrhoids

Abstract:
Objective To explore the effects of Huayu Sanjie decoction combined with microwave on postoperative pain mediators and the levels of serum chemokine receptor 4 (CXCR4), chemokine ligand 12 (CXCL12), and chemokine receptor 7 (CXCR7) in patients with circular mixed hemorrhoids. Methods From November 2023 to August 2024, 78 patients with ring mixed hemorrhoids who underwent surgical treatment were grouped into a routine group (39 cases, microwave) and a union group (39 cases, additionally used Huayu Sanjie decoction) via random number table. The pre- and post-treatment TCM syndrome scores, local condition of the anus, quality of life, pain mediators, and serum levels of CXCR4, CXCL12, CXCR7 were compared, and wound healing time were evaluated. Results The postoperative wound healing time of the combined group was significantly shorter than that of the conventional group. The wound inflammation exudation scores on the 14th and 42nd days after surgery were lower in the combined group than in the conventional group (P <0.05). On the 7th day after surgery, the TCM syndrome score of the combined group was lower than that of the conventional group, and the GQOL-74 score was higher than that of the conventional group (P <0.05). On the 7th and 14th days after surgery, the TCM syndrome score, visual analogue scale (VAS), prostaglandin E2 (PGE2), bradykinin (BK), and CXCR4 levels of the combined group were all lower than those of the conventional group (P <0.05). The serum CXCL12 and CXCR7 levels of the combined group were higher than those of the conventional group (P <0.05). Conclusion The union of Huayu Sanjie decoction and microwave therapy for patients with ring mixed hemorrhoids can help speed up wound healing after surgery and effectively reduce pain mediators and inflammatory factors.
