Effects of modified Simo decoction combined with esomeprazole on wound healing in high-grade intraepithelial neoplasia of gastric mucosa after endoscopic submucosal dissection

Abstract:
Objective To explore the effects of modified Simo decoction combined with esomeprazole on wound healing in high-grade intraepithelial neoplasia of gastric mucosa after endoscopic submucosal dissection (ESD). Methods A total of 88 patients with high-grade intraepithelial neoplasia of gastric mucosa undergoing gastric ESD in the hospital were enrolled between February 2023 and April 2025. According to random number table method, they were divided into control group (esomeprazole treatment from 3 d after surgery, n=44) and treatment group (modified Simo decoction on basis of control group, n=44). After 4 weeks of treatment, curative effect, scores of TCM syndromes, wound healing and complications were compared between the two groups, and levels of serum C-reactive protein (CRP) and pepsinogen I/pepsinogen II (PG I/PG II) were detected. Results The curative effect of treatment group was better than that of control group(95.45% vs 76.19%), the difference was statistically significant (P <0.05), wound healing rate was higher than that in control group, and the disappearance time of upper abdominal symptoms was shorter than that in control group, and the differences were statistically significant (P <0.05). After treatment, scores of TCM syndromes and level of serum CRP were decreased in both groups, which were lower in treatment group than control group, the differences were statistically significant (P <0.05). After treatment, levels of serum PG I/PG II were increased in both groups, which were higher in treatment group than control group, the differences were statistically significant (P <0.05). The difference in the occurrence of drug-related adverse reactions, delayed bleeding and gastric perforation between the two groups was not statistically significant (P >0.05). Conclusion Modified Simo decoction combined with esomeprazole can increase wound healing rate, effectively reduce inflammatory response, promote gastric mucosa repair and alleviate clinical symptoms in high-grade intraepithelial neoplasia of gastric mucosa after ESD.
