2022—2024年天津市南开医院临床血标本中病原菌分布及菌株的耐药性分析

摘要:
目的:分析天津市南开医院临床血标本中病原菌分布和菌株的耐药情况,为合理使用临床抗菌药物提供依据。方法:应用全自动血培养仪对2022—2024年天津市南开医院40 251例临床送检的血标本进行病原菌培养;经Vitek MS全自动微生物检测系统VITEK 2 compact全自动微生物鉴定及药敏分析系统对病原菌进行鉴定和药敏实验,按照美国临床标准化委员会(CLSI)标准判定药敏结果。结果:40 251例临床血标本中,污染率1.65%(665/40 251);分离出病原菌4205株,其中革兰阳性菌2120株、革兰阴性菌1749株、真菌174株及厌氧菌162株。病原菌主要分布于肝胆胰外科、重症医学科(外科)、胃肠外科、重症医学科(内科)、肿瘤内科、呼吸与危重症科、感染性疾病科。革兰阳性菌以凝固酶阴性葡萄球菌和肠道球菌为主;革兰阴性菌以大肠埃希菌、肺炎克雷伯菌、阴沟肠杆菌、铜绿假单胞菌、鲍曼不动杆菌为主。革兰阳性菌对万古霉素、替加环素和利奈唑胺较敏感,发现1例耐万古霉素的屎肠球菌;革兰阴性菌不同程度地对碳青霉烯类抗生素耐药。结论:血培养分离的病原菌种类多,革兰阴性菌对常见抗菌药物耐药情况严重,尤其是耐碳青霉烯类肺炎克雷伯菌(CR-KP)。临床应加强血培养送检质量,监测病原菌分布及耐药性,合理使用抗菌药物。

Abstract:
Objective To investigate the distribution and drug resistance of pathogens isolated from blood culture from 2022 to 2024, and to provide basis for clinical drug use. Methods 40 251 blood specimens from 2022 to 2024 were cultivated with automated blood culture systems; bacteria identification and drug resistance were carried by Vitek MS and compact bacteria identification system, with susceptibility results interpreted according to CLSI standards. Results There were 40 251 blood specimens, with a contamination rate of 1.65%(665/40 251). 4205 strains of pathogens were isolated, including 2120 gram-positive bacteria, 1749 gram-negative bacteria, 174 fungi and 162 anaerobic bacteria. The pathogens were mainly distributed in Hepatobiliary and Pancreatic Surgery Department, ICU, Gastrointestinal Surgery Department, EICU, Medical oncology Department, Respiratory and Critical Care Department, and Infectious Diseases Department. Mainly gram positive cocci were coagulase negative staphylococcus and Enterococcus; gram negative bacteria were mainly Escherichia coli, Klebsiella pneumoniae, enterobacter cloacae, Pseudomonas aeruginosa, Acinetobacter baumannii, who were resistant to carbapenems. Gram positive cocci were sensitive to vancomycin, tigecycline and linezolid, and 1 Enterococcus faecium was resistant to vancomycin; gram negative bacteria exhibit varying degrees of resistance to carbapenem antibiotics. Conclusion There are many types of pathogens isolated and gram negative bacteria have severe resistance to common antibiotics, especially carbapenem-resistant Klebsiella pneumoniae. The distribution of pathogens and drug resistance should be strengthened to provide an optimal antimicrobial therapy.
