Effect of Bugu capsule on postoperative rehabilitation and expression of serum bone markers P1NP and β-CTx in patients with upper limb fracture

Abstract:
Objective To observe the clinical efficacy of Bugu capsule in postoperative rehabilitation of patients with upper limb fracture (ULF) and its influence on serum bone turnover markers—procollagen type I N-terminal propeptide (P1NP) and β-crosslaps (β-CTx). Methods A total of 120 patients who underwent ULF surgery at the Department of Orthopaedic Trauma, Chongqing Traditional Chinese Medicine Orthopaedic Hospital, between January and December 2024 were enrolled. They were equally randomized into a control group and an observation group (n=60 each) using a random-number table. Both groups were managed with an identical clinical pathway of homogeneous internal fixation. Starting 1 week post-operatively, the control group received oral calcium carbonate alone, whereas the observation group received calcium carbonate plus Bugu capsule. After 8 weeks of treatment, the excellent-and-good rate of clinical recovery, Traditional Chinese Medicine(TCM) symptom scores, local fracture findings, Fugl-Meyer upper-extremity motor scores, DASH upper-limb disability scores, serum P1NP and β-CTx levels, and incidence of complications were compared between the two groups. Results The excellent and good rate of clinical treatment in the experimental group was significantly higher than that in the control group, the reduction of TCM symptom score was better than that in the control group, and the local pain, swelling disappearance time and fracture healing time in the experimental group were less than those in the control group (P <0.05). The Fugl-Meyer score in the experimental group was significantly higher than that in the control group, and the DASH score was significantly lower than that in the control group (P <0.05). The expression levels of serum P1NP and β-CTx in the experimental group were significantly lower than those in the control group (P <0.05). There was no significant difference in the incidence of complications between the two groups (P >0.05). Conclusion Bugu capsule has good rehabilitation effect after ULF, which can relieve local swelling and pain after operation, shorten fracture healing time, reduce the upper limb obstacle rate, improve the expression of serum bone markers, and has good safety, without special complications.
