| 摘要: |
| 目的:评价清热利湿合剂联合黄连消肿膏外敷治疗湿热瘀阻型血栓性浅静脉炎(SVT)的临床效果。方法:选取2023年5月—2024年8月天津中医药大学第一附属医院收治的湿热瘀阻型SVT患者100例，根据是否接受研究方案，分为对照组(48例)和观察组(52例)两组。对照组予基础治疗，观察组在基础治疗上，予以清热利湿合剂口服联合黄连消肿膏外敷，治疗周期为10 d，比较两组患者的中医症状评分、临床总有效率、白细胞数(WBC)、C-反应蛋白(CRP)、降钙素原(PCT)、D-二聚体(DD)及凝血指标、疼痛NRS评分。结果:治疗10 d后，观察组患者的皮肤颜色、条索状物评分、肢体肿胀、疼痛评分低于对照组(P ＜0.05)；总有效率为86.54%，高于对照组(P ＜0.05)；CRP、WBC、活化部分凝血活酶时间(APTT)低于对照组(P ＜0.05)；观察组患者的NRS评分为(3.15±0.60)分，低于对照组的(3.44±0.31)分，差异有统计学意义(P ＜0.05)。结论:清热利湿合剂联合黄连消肿膏是治疗湿热瘀阻型SVT的有效手段。 |

| Objective To evaluate the effectiveness of Qingre Lishi mixture combined with Huanglian Xiaozhong cream of superficial vein thrombophlebitis of Shire Yuzu type. Methods 100 patients admitted from the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine from May 2023 to August 2024, A cohort study based on study protocol treatment as an exposure factor. The control group (non-exposed group) was treated with basic western medicine, In the observation group (exposure group), on the basic treatment of western medicine, Qingre Lishi mixture combined with Huanglian Xiaozhong cream. The treatment cycle was 10 days, Collection of general data, Investigate the TCM symptom score, clinical total response rate, and white blood cell number (WBC), C-reactive proteins(CRP), procalcitonin(PCT), D-dimer(DD), and coagulation markers, and pain NRS score. Results After 10 days of treatment, skin color, bar score, limb swelling and pain were lower than observation group (P ＜0.05); total response rate was 86.54% higher than control group; CRP and WBC lower (P ＜0.05), PCT was not statistically significant; APTT lower than control group (P ＜0.05); NRS score was (3.15±0.60) lower than observation group (3.44±0.31,P ＜0.05). Conclusion It is an effective method of Qingre Lishi mixture combined with Huanglian Xiaozhong cream for superficial vein thrombophlebitis of Shire Yuzu type. |
