脓毒症急性肺损伤患者的代谢组学变化分析

摘要:
目的:利用非靶向代谢组学寻找脓毒症伴急性肺损伤(ALI)特异性代谢标志物和代谢途径,为临床诊疗提供新的依据。方法:采用超高效液相色谱/质谱联用仪分别对脓毒症伴ALI组(23例)、脓毒症不伴ALI组(14例)及健康对照组(15例)的血清样本进行代谢组学检测,并对差异代谢物及代谢途径进行统计分析。结果:与脓毒症不伴ALI组比,脓毒症伴ALI组的炎症相关代谢物7-氧代胆固醇、马尿酸浓度增加,抗氧化代谢物马特果酚7-O-葡萄糖苷、扁柏酚葡萄糖苷、去甲基阿兰吉苷浓度增加,能量相关代谢物PGK1、DL-3-羟基己酸和异纤维蛋白B3浓度增加,抗炎代谢物甘氨脱氧胆酸、抗氧化代谢物2-羟基雌酮、内源性代谢物N-(3-氯苯甲酰基)甘氨酸及药物(M)-生物碱浓度均降低。显著变化的代谢途径有雌醇代谢、氨回收、组氨酸代谢和胆汁酸代谢。结论:用非靶向代谢组学方法在脓毒症伴ALI患者血清中筛选出具有显著变化的炎症相关代谢物、抗氧化代谢物、能量相关代谢物及抗炎代谢物等,该组患者存在雌醇代谢、氨回收、组氨酸代谢和胆汁酸代谢途径紊乱。

Metabolomic clinical analysis of sepsis combined with acute lung injury

Abstract:
Objectives Using non-targeted metabolomics methods to search for specific metabolite markers and metabolism pathways in sepsis with acute lung injury (ALI), providing new basis and evidence for clinical diagnosis and treatment. Methods Metabolomics analysis was performed on serum samples from three groups of subjects with sepsis accompanied by ALI (23 cases), without ALI (14 cases), and healthy individuals (15 cases) during the same period using ultra-high performance liquid chromatography/mass spectrometry(UPLC/MS). Differential metabolites and metabolism pathways were statistically analyzed. Results Compared with the sepsis without ALI group, the sepsis with ALI group showed an increase in the concentrations of inflammation related metabolites 7-oxocholesterol and hippuric acid, antioxidant metabolites 7-O-glucoside, kaempferol glucoside, and demethylangin, energy related metabolites PGK1, DL-3-hydroxyhexanoic acid, and fibrinogen B3, and a decrease in the concentrations of anti-inflammatory metabolites glycerodeoxycholic acid, antioxidant metabolite 2-hydroxyestrone, endogenous metabolite N-(3-chlorobenzoyl) glycine, and drug (M) - alkaloids. The metabolism pathways with significant changes include estradiol metabolism, ammonia recovery, histidine metabolism, and bile acid metabolism. Conclusion Non-targeted metabolomics methods were used to screen for inflammation related metabolites, antioxidant metabolites, energy related metabolites, and anti-inflammatory metabolites with significant changes in the serum of sepsis with ALI patients. At the same time, there were disturbances in estradiol metabolism, ammonia recovery, histidine metabolism, and bile acid metabolism pathways in this group of patients.
