目的:研究芪参还五胶囊联合穴位贴敷对中重度肘管综合征患者术后尺神经及手功能恢复的临床疗效。方法:选择2022年8月—2024年8月在河北省沧州中西医结合医院行尺神经松解术治疗的100例中重度肘管综合征患者,采用随机数字法分为观察组和对照组,每组各50例。对照组术后使用甲钴胺片治疗,观察组术后给予甲钴胺片加用芪参还五胶囊联合穴位贴敷治疗。治疗3个月后,比较两组患者的尺神经运动传导速度、环小指两点辨别觉、拇示指正向捏力、拇示指侧方捏力和握力。结果:术后3个月,观察组患者尺神经运动传导速度高于对照组[(46.06±8.04)m/s vs (42.28±8.22)m/s];环指、小指两点辨别觉数值低于对照组[(12.04±2.54)mm、(12.06±2.17)mm vs (13.24±2.82)mm、(13.14±3.15)mm];拇示指正向捏力大于对照组[(5.33±2.07)N vs (4.56±1.66)N];拇示指侧方捏力大于对照组[(6.18±2.10)N vs (5.29±1.69)N];握力大于对照组[(28.15±6.02)N vs (25.09±7.56)N],且观察组总体优良率高于对照组(96% vs 84%),差异均有统计学意义(P <0.05)。结论:芪参还五胶囊联合穴位贴敷可促进肘管综合征患者术后手部感觉及运动功能恢复,具有较好的临床疗效。
Clinical efficacy of Qishen Huanwu capsule combined with acupoint application on postoperative hand function recovery in patients with cubital tunnel syndrome

Abstract:
Objective To investigate the clinical efficacy of Qishen Huanwu Capsules combined with acupoint application in the recovery of ulnar nerve and hand function after surgery for moderate to severe cubital tunnel syndrome. Methods 100 patients with moderate to severe cubital tunnel syndrome who underwent ulnar nerve release surgery at Cangzhou Hospital of Traditional Chinese and Western Medicine in Hebei Province from August 2022 to August 2024 were randomly divided into an observation group and a control group, with 50 patients in each group. The control group was treated with mecobalamin tablets, while the observation group was treated with mecobalamin tablets combined with Qishen Huanwu capsules and acupoint application after surgery. After 3 months of treatment, the ulnar nerve motor conduction velocity, two-point discrimination sensation of the ring little finger, positive pinch force of the thumb index finger, lateral pinch force of the thumb index finger, and grip force were compared between the two groups of patients. Results The ulnar nerve motor conduction velocity in the observation group was higher than that in the control group [(46.06±8.04)m/s vs (42.28±8.22)m/s]; The discrimination values of the ring finger and little finger were lower than those of the control group [(12.04±2.54)mm, (12.06±2.17)mm vs (13.24±2.82)mm, (13.14±3.15)mm]; The positive pinching force of the thumb index finger was stronger than that of the control group [(5.33±2.07)N vs (4.56±1.66)N]; The lateral pinching force of the thumb index finger was stronger than that of the control group [(6.18±2.10)N vs (5.29±1.69)N]; The grip strength was stronger than that of the control group [(28.15±6.02)N vs (25.09±7.56)N], and the overall excellent and good rate of the observation group was higher than that of the control group (96% vs 84%), with statistical significance (P <0.05). Conclusion The combination of Qishen Huanwu capsules and acupoint application can promote postoperative sensory and motor function recovery in patients with cubital tunnel syndrome, and has good clinical efficacy.
