目的 研究结肠镜检查前使用复方聚乙二醇电解质散(PEG)与硫酸镁对肠道清洁程度和患者依从性的影响。方法:回顾性选取2023年1月—2023年12月就诊于天津市南开医院内镜中心、经社区筛查便潜血呈阳性需进一步行结肠镜检查的患者作为研究对象。根据患者使用不同的肠道清洁剂分为PEG组(n=500)和硫酸镁组(n=500)，使用波士顿肠道准备量表(BBPS)评估两组患者肠道准备质量，采用视野清晰度评分法评估肠道气泡量，采用问卷调查法对患者依从性和舒适度进行评分。结果:与PEG组比较，硫酸镁组BBPS评分升高，气泡量减少，差异有统计学意义(P<0.05)。PEG组的总依从度高于硫酸镁组(95% vs 74%)，差异有统计学意义(P<0.05)。硫酸镁组口服药物及水的总量显著高于PEG组，硫酸镁的口感比PEG更难接受，患者服用硫酸镁后排便显著频繁，中重度腹痛患者比例较高，总睡眠时间显著降低，睡眠期间清醒次数显著增多，入睡后清醒时间显著增长，差异有统计学意义。结论:PEG与硫酸镁各有优缺点，硫酸镁肠道清洁效果好，镜下清晰，但患者安全性、舒适度和依从性低于PEG。
Comparison of intestinal cleanliness and patient compliance between magnesium sulfate and compound polyethylene glycol electrolyte powder in colonoscopy examination

Abstract:
Objective To investigate the effects of compound polyethylene glycol electrolyte powder (PEG) and magnesium sulfate on bowel cleansing efficacy and patient compliance. Methods Patients undergoing colonoscopy at Nankai Hospital, Tianjin, from January 2023 to December 2023, were retrospectively selected as study subjects. They were divided into two groups based on the bowel preparation agent used: the PEG group (n=500) and the magnesium sulfate group (n=500). The quality of bowel preparation was assessed using the Boston Bowel Preparation Scale (BBPS). Additionally, the clarity of the visual field was evaluated using a bubble scoring method, while patient compliance and comfort were assessed through questionnaires. Results Compared to the PEG group, the magnesium sulfate group showed significantly higher BBPS scores and reduced bubble presence (P<0.05). The overall compliance of the PEG group was higher than that of the magnesium sulfate group (95% vs 74%，P<0.05). However, patients in the magnesium sulfate group reported poorer taste, higher oral medication and water intake, more severe abdominal pain and bloating, increased frequency of bowel movements, reduced sleep duration, and more sleep disturbances (P<0.05). Conclusion Both PEG and magnesium sulfate have their advantages and limitations. Magnesium sulfate provides superior bowel cleansing and clearer visual fields during colonoscopy but is associated with lower patient safety, comfort, and compliance compared to PEG. PEG, while gentler and more palatable, results in more bubbles under the microscope and slightly lower cleansing efficacy. For elderly or frail patients with underlying health conditions, PEG may be a preferable option for bowel preparation.
