肺部微生物与肺癌相关性的研究进展

摘要
肺癌是世界范围内癌症相关死亡的主要原因,通常是宿主、微生物以及环境间动态平衡失调的不良结果。微生物组特征在多种肿瘤的发展中具有显著的影响,通过宿主的免疫系统发挥作用而成为癌症生长可能的致病因素。最近有研究专注于微生物如何响应肺癌,特别是肺的局部微生物群落如何塑造肿瘤形成微环境并促进肺肿瘤进展,提示肺部微生物与肺癌的发生、发展及治疗有关。本文就肺部微生物在肺癌中的研究进展作一综述。
肿瘤是一种由宿主和环境因素相互作用引起的复杂疾病。微生物在维持微生物组平衡和调节宿主免疫反应等多种方面发挥着重要作用。肺癌是最常见的癌症之一,也是全世界癌症相关死亡的主要原因,估计每年新增200万例,死亡病例每年可达1.76亿[1-2]。肺癌包括非小细胞肺癌(non-small cell lung cancer,NSCLC)和小细胞肺癌,其中NSCLC占85%以上[3]。近年来研究发现,肺部微生物的生态平衡破坏与肺癌进展密切相关[4-5]。本文就肺微生物组与肺癌之间相互作用和潜在机制,以及改变肺微生物组治疗肺癌的相关研究进展进行综述。
1 肺部微生物构成的意义
过去学术界普遍认为,健康的肺部是“相对无菌”的,而当前研究则认为健康肺部同样存在低丰度的微生物菌群,其组成与上呼吸道非常相似[6-7]。绝大部分肺部微生物群是通过吸入上呼吸道中的微量细菌而被动产生的。Bassis等[8]比较了健康成年人口腔、鼻腔、肺和胃中的微生物组成,发现胃液和肺泡灌洗液中的微生物群落主要来源于口咽菌群的吸入和定植。更有研究发现,出生、出生后的第1个小时以及暴露在生活环境中的前3~4个月是肺部菌群稳态形成的重要阶段[9]。在健康的肺部中,微生物的种类及丰度维持相对稳定的状态,最常见的是厚壁菌门和拟杆菌门,而链球菌属和韦氏菌属在其中占优势[710]。而在患病患者的肺中,局部环境改变、纤毛功能失调、黏液分泌增加或细菌迁移及增殖增加会导致局部微生物载量增加及种类组成改变[11]。从生理学的角度来看,肺部微生物对肺组织具有双重影响:一方面,微生物可协助宿主调控免疫应答水平,使宿主免疫系统对病原体定植的抵抗力和对自身成分或非危险异物的耐受力形成微妙平衡;另一方面,肺部微生物会促进肺部疾病的发生、发展[12-14]。已有研究表明,微生物与各种肺部疾病如哮喘[15-16]、慢性阻塞性肺病[17-19]和癌症[20-21]之间存在联系。
2 肺部微生物与肺癌
2.1 肺部微生物与肺癌的发生、发展
已有研究表明,肺部微生物群紊乱可诱导局部炎症因子的释放,诱导局部慢性炎症环境,刺激气道上皮细胞的增殖,最终诱发细胞转化,启动肿瘤形成并促进肿瘤演进[5]。结核分枝杆菌与肺癌之间具有显著关联是上述慢行炎症-肿瘤互作理论的典型例子。结核分枝杆菌可诱导肿瘤坏死因子的产生,诱导肺部的产生慢性炎症,介导肿瘤发生发展;而肺癌患者化疗或放疗后免疫力低下,有助于结核菌增殖,增加了结核感染的风险[22]。其他慢性肺部炎症,如慢性阻塞性肺病,已被认为是NSCLC的常见危险因素,同样是肺部微生物紊乱介导的炎症促进的肺癌进展的重要证据。此外,微生物群失调等因素会影响微生物毒素和代谢物的产生,而上述物质同样参与了癌症的发生发展。细菌产生的毒素和超氧化物可直接损伤细胞DNA,介导肿瘤细胞产生基因突变。已有充分的研究证明,大肠杆菌属pks+ E.coli产生的大肠杆菌素可诱导体细胞出现双链断裂和其他形式的DNA损伤,在体外和体内条件下都可介导结直肠癌中出现独特的突变特征[23]。有研究发现,肺癌组织内Acidovorax菌和Massilia菌的丰度与肺癌P53基因突变水平显著相关[24]。在肺腺癌中,Parvimonas菌与较晚的肺癌分期及肿瘤出现人表皮生长因子受体(EGFR)突变有显著相关性[25]。不仅如此,肺癌相关微生物可通过产生小分子代谢物,调控肿瘤表观遗传学改变,从而促进肺癌进展。肺炎毒素、肺炎球菌溶血素和丙酮酸氧化酶可以通过宿主PP1磷酸酶使组蛋白H3发生去磷酸化,从而增强肺癌细胞的增殖能力[26]。肺炎链球菌、变形菌门、拟杆菌门、厚壁菌门和放线菌门等肺部常见菌种可产生丁酸盐、丙酸盐、叶酸盐、乙酸盐等代谢物,而上述分子已被证明可以诱导肿瘤细胞出现表观遗传学改变,促进肿瘤细胞进展[27]。细菌脂多糖激活的Toll样受体4(TLR4)可导致核因子(NF)-κB蛋白入细胞核参与转录,导致各种炎症相关基因的表达[28]。这些研究说明,肺部微生物可通过参与塑造免疫微环境、介导肿瘤细胞发生遗传突变、调控肿瘤细胞基因表达水平等多种途径,推动肿瘤演进过程,提示肺部菌群紊乱对肺癌发生、发展具有重要意义。
2.2 肺癌的微生物组特征
肺癌患者的肺部微生物构成和丰度不同于正常人群。与健康肺组织相比,肺癌组织及癌旁组织的的生境内多样性(α多样性)显著升高[27]。癌症患者中最常见的肺部微生物门依然是厚壁菌门,其次为变形杆菌门和拟杆菌门,普雷沃菌属替代链球菌属和韦氏菌属成为厚壁菌门中最常见的属[29-30]。链球菌主要与小细胞肺癌组织学类型相关[31],有研究表明,癌症患者下呼吸道的支气管镜样本中链球菌显著增多[32],因此有部分学者认为,肺中链球菌的丰度可以作为肺腺癌转移的预测因子[33]。在NSCLC病例中,韦氏菌属丰度显著升高。Thai等[34]强调,韦氏菌属Veillonella可以与Acidovorax一起用作肺鳞状细胞癌的生物标志物。在拟杆菌门的微生物中,Capnocytophaga属和Prevotella属是最常见的可鉴定属。有研究已经报道肺腺癌患者肺组织可检测出Capnocytophaga[35-36]。有人建议,痰中Capnocytophaga与假单胞菌科家族的微生物可联合作为肺癌的诊断生物标志物,而另一项研究也建议将Capnocytophaga视为诊断肺腺癌的生物标记物[3437]。据报道,Ⅳ期肺腺癌患者肺内的Capnocytophaga丰度升高,而在另一项研究中,在肺鳞癌患者肺组织内也发现了类似情况[32-33]。近年来多项报道指出肺癌患者肺组织中变形杆菌门的多种菌属的丰度出现了显著变化。在患有转移性肺癌患者的肺组织内,Legionella属的丰度显著升高[38];而在其他研究中,Rothia属丰度在肺腺癌患者肺组织内显著降低[32]。在肺癌患者的唾液中,发现变形杆菌门的其他菌属丰度出现明显变化,如在NSCLC患者唾液中的Lautropia,以及在女性非吸烟者NSCLC患者唾液中的Blastomonas属和Sphingomonas属的丰度均较对照组出现显著升高[39-40]。然而,与患有肺气肿但没有肺癌的重度吸烟者相比,具有吸烟史的肺癌患者的肺部细菌群落结构具有显著差异,变形杆菌门显著减少,厚壁菌门和拟杆菌门则显著增多[41]。一项使用5篇针对肺组织样本进行微生物测序的荟萃分析同样揭示了一些相互矛盾的结果。变形杆菌在癌症患者的组织中不占优势,尤其是变形杆菌属的盐单胞菌在癌症中的相对丰度显著降低[42]。造成上述矛盾结果的原因可能是使用不同的样本采集方法和样本类型以及缺乏标准化分析等原因导致的,未来肺部组织内变形杆菌门丰度的改变与肺癌临床特点的相关性还需要进一步分析论证。总之,肺部微生物群失衡在肺癌的发生和发展中起着至关重要的作用。
3 肺部微生物与肺癌的免疫治疗
免疫治疗耐受性良好且疗效持久,是近年来肺癌治疗的突破性成果,因此更好地了解微生物对免疫治疗疗效和毒性的影响成为至关重要的研究课题。已有研究揭示滥用抗生素影响肺癌免疫治疗效果:免疫治疗前第1个月使用抗生素会加速患者肺功能衰竭,显著缩短患者的生存时间[43]。一项荟萃分析则表明,在NSCLC患者接受免疫检查点抑制剂(immune checkpoint inhibitor,ICI)治疗之前或者治疗前期使用抗生素,患者中位总生存期的中位数减少6个月以上,而在ICI疗程后期使用抗生素似乎不会改变存活率。不仅如此,接受抗生素治疗的肺癌患者在接受ICI治疗时发生免疫相关不良事件的潜在风险明显较高,这可能与使用抗生素导致微生物多样性降低相关[44]。尽管肺部微生物组的巨大潜力为癌症的预防和治疗开辟了广阔的前景,但人们普遍认为,该领域的发展需要更多的多学科和深入的探索。随着癌症的进展和对多种治疗的不同反应,更好地了解微生物组可能会为癌症患者的诊断和预后提供巨大的机会。
4 小结
目前,肺部微生物组与肺癌发生和进展的关系还处于早期阶段,关于其参与肺疾病的机制研究甚至更少。然而,大量证据支持局部微生物群参与肺癌的发生、发展和治疗反应。因此,深入研究人类微生物群与肺癌细胞之间的相互作用,以及识别参与这种相互作用的细胞和分子介质,可成为揭示肺癌发生、发展机制新的切入点。同时,微生物组还对肺癌化疗、靶向治疗和免疫治疗的效果有重要影响,有望为未来肺癌的治疗提供新策略。
