胫骨高位截骨术远端螺钉不同长度置钉的生物力学分析

摘要:
目的:研究分析胫骨高位截骨术(HTO)中内固定最远端两颗螺钉不同长度配置对其生物力学性能的影响。方法:选取1例膝关节骨关节炎患者,进行下肢全场64排螺旋CT扫描获得所需数据,采用有限元分析技术构建基于Tomofix钢板与远端使用不同长度螺钉固定,对胫骨平台表面施加3倍体重的载荷,比较不同模型应力与位移变化情况。结果:结合应力与位移数据,次远端单皮质置钉的螺钉应力峰值为348.7 MPa,与正常置钉相近,胫骨应力峰值为98.7 MPa,在所有置钉方式中最低,位移形变15.63 mm,控制良好。其他方式比较,远端或次远端不置钉时胫骨和螺钉应力形变显著升高,风险增加。结论:次远端单皮质置钉在平衡内固定与骨骼负载方面表现最佳,为最优置钉方式。HTO手术时钢板远端使用单皮质螺钉具有可行性。

Biomechanical analysis of different lengths of distal screws in high tibial osteotomy

Abstract:
Objective To analyse the effect of different length of the two distal screws of internal fixation on the biomechanical properties in high tibial osteotomy (HTO). Methods Selecting one patient with knee osteoarthritis and perform a 64-slice spiral CT scan of the lower extremity to obtain the required data. Using finite element analysis techniques, the Tomofix-based steel plate was constructed and the distal end of the plate was fixed using screws of different lengths, and a load of three times the body weight was applied to the surface of the tibial plateau to compare the changes in stress and displacement in different models. Results Combining the stress and displacement data, the sub-distal single cortical nailing had a peak screw stress of 348.7 MPa, which was similar to the normal nailing, and a peak tibial stress of 98.7 MPa, which was the lowest among all the nailing modalities, with a displacement deformation of 15.63 mm, which was well controlled. Tibial and screw stress deformations were significantly higher with distal or subdistal non-nailing than with other modalities, with increased risk. Conclusion The subdistal single cortical nailing performed best in balancing internal fixation and bone loading, and was the optimal nailing method. Taken together, the use of single-cortical screws at the distal end of the plate during HTO surgery is feasible.
