| 目的:观察中药膏摩八髎穴治疗老年混合痔术后患者合并脾虚气弱型便秘的临床效果。方法:选择2022年6月—2023年9月在首都医科大学附属北京中医医院肛肠科行混合痔手术治疗并伴有脾虚气弱型便秘的老年患者120例，按照随机数字表法分为治疗组和对照组，每组各60例。对照组予聚乙二醇4000散口服，每次1包，每天2次。治疗组在对照组基础上给予八髎穴中药膏摩疗法。治疗2周后对两组患者临床疗效进行评价，比较两组治疗后便秘严重程度评分(CSS)、生活质量量表(PAC-QOL)评分、平均每周自发完全排便次数(SCBM)、便时肛门疼痛评分、肛缘水肿评分及结肠传输试验。结果:治疗组总有效率为91.67%，高于对照组78.33%，差异有统计学意义(P ＜0.05)。治疗后，治疗组患者的SCBM高于对照组，CSS、便时肛门疼痛及肛缘水肿评分低于对照组(P ＜0.05)。治疗后，治疗组患者生活质量评分低于对照组，且治疗组患者72 h肠道残留标记物数量明显低于对照组(P ＜0.05)。结论:中药膏摩八髎穴治疗老年混合痔术后患者合并脾虚气弱型便秘效果显著，能够促进相关临床症状消退，提升临床疗效，减轻患者焦虑抑郁情绪，提升其术后生活质量。 |

| Clinical effect of traditional Chinese medicine ointment at the Baliao acupoint in the treatment of elderly patients with mixed hemorrhoids complicated by spleen deficiency and weak Qi constipation after surgery |

| Objective To explore the clinical efficacy of traditional Chinese medicine ointment at the Baliao acupoint in treating constipation with spleen deficiency and weak Qi in elderly patients after mixed hemorrhoid surgery. Methods 120 elderly patients who underwent mixed hemorrhoid surgery and were accompanied by spleen deficiency and weak qi constipation in the Colorectal Department of Beijing Traditional Chinese Medicine Hospital from June 2022 to September 2023 were randomly divided into a treatment group and a control group, with 60 patients in each group. The control group was given polyethylene glycol 4000 powder orally, 1 pack/time, 2 times/day. The treatment group was given Baliao acupoint ointment massage therapy on the basis of the control group. Treatment for 2 weeks. Evaluate the clinical efficacy of two groups of patients after treatment, observe and compare the post-treatment (CSS), PAC-QOL scores, weekly spontaneous complete bowel movements (SCBM), anal pain during defecation, anal margin edema, and colon transit test between the two groups. Results The total effective rate of the treatment group was 91.67%, which was higher than the control group's 78.33%(P ＜0.05). After treatment, the SCBM of the treatment group patients was higher than that of the control group (P <0.05), while the CSS, anal pain during defecation, and anal margin edema of the treatment group patients were lower than those of the control group (P <0.05). The quality of life score of the treatment group was lower than that of the control group (P <0.05). The residual markers in the intestinal tract of patients treated for 72 hours were significantly lower than those in the control group (P <0.05). Conclusion The use of Baliao acupoint has shown significant efficacy in treating elderly patients with mixed hemorrhoids who suffer from spleen deficiency and weak Qi constipation after surgery, improving their postoperative quality of life. |
