Clinical study on the treatment of postoperative wound healing disorderin calcaneal fracture with Buyang Huanwu decoction

Abstract:
Objective To evaluate the efficacy of Buyang Huanwu decoction in preventing delayed healing of incisions after calcaneal fracture surgery. Methods From October 2021 to December 2023, 60 patients with calcaneal fractures were enrolled and randomly divided into a treatment group (n=30) receiving Buyang Huanwu decoction and noscapine, including 29 males and 1 female; and a control group (n=30) receiving noscapine only,including 28 males and 2 females. All patients underwent a lateral L-shaped incision for open reduction and internal fixation with plates and screws. Routine drainage was performed for 48 hours postoperatively, and data on patient gender, age, fracture classification, operation time, postoperative ankle circumference difference, serum interleukin-1 (IL-1), basic fibroblast growth factor (bFGF), CRP levels, and incision healing grade were recorded.Statistical analysis was conducted. Results There were no statistically significant differences between the two groups regarding gender, age, fracture classification, and operation time. On postoperative days 5 and 7, the ankle circumference difference values in the treatment group were (4.06±0.83) cm and (3.42±0.66)cm, were lower than those in the control group (4.71±0.87)cm and (3.93±0.75)cm, which was statistically significant (p<0.05). On postoperative day 7, the level of IL-1 in the treatment group (31.68±3.54)ug/L was significantly lower than that in the control group (34.13±3.85)ug/L. On postoperative day 7,the level of CRP in the treatment group (19.51±2.57)mg/L was significantly lower than that in the control group (21.36±2.64) mg/L, showing a statistical difference (p<0.05). Similarly, the level of bFGF on postoperative day 7 in the treatment group (236.94±8.25) pg/mL was notably higher than that in the control group (229.74±11.35) pg/mL, also with a significant statistical difference (p<0.05). The healing rate of the treatment group was higher than that of the control group (96.67% vs 83.33%, p=0.025). Conclusion The application of Buyang Huanwu decoction combined with noscapine postoperatively in calcaneal fractures can promote incisional healing and reduce the occurrence of complications.
