Baitouweng decoction enema for ulcerative colitis: a system review and Meta-analysis

Abstract:
Objective To evaluate the efficacy and safety of Baitouweng decoction enema in the adjuvant treatment of ulcerative colitis (UC). Methods Randomized controlled trials were searched in China Academic Journal Full-text Database (CNKI), China Biomedical Literature Database (CBM), VIP database, Wanfang database, clinical trial registry, PubMed, Embase etc. The Meta-analysis of outcome index data was performed using RevMan 5.3 software. Results Fourteen studies were selected, and the results of Meta-analysis showed that the total response rates were as follows[RR=1.21, 95%CI =(1.15,1.28), P<0.01]. Incidence of adverse reactions [RR=0.50,95%CI=(0.27,0.92), P<0.01]; Abdominal pain score [SMD=-1.05, 95%CI= (-1.48,-0.62), P<0.01]; Diarrhea score comparison [SMD=-1.32, 95%CI= (-2.00,-0.65), P<0.01]; [SMD=-2.95, 95%CI= (-4.60,-1.29), P<0.01]; Abscess, blood and stool score: [SMD=-1.91, 95%CI= (-2.99,-0.83), P<0.01]; mayo score: [MD=-1.22<95, 95%CI=(-1.85,-0.59), P<0.001]; Comparison of Baron scores [MD=-0.30, 95%CI= (-0.45,-0.16), P<0.01]; Level of IL-10 SMD=1.17, 95%CI= (0.90,1.43), P<0.01; TNF-α level [SMD=-2.20, 95%CI= (-3.48,-0.92), P<0.001]; TNF-β level [SMD=-1.54, 95%CI=(-1.89,-1.18), P<0.01]. Conclusion The enema with Baitouweng decoction can improve the total therapeutic effect, relieve the symptoms of abdominal pain, diarrhea and purulent bloody stool in UC patients, inhibit intestinal inflammatory reaction and promote the healing of intestinal mucosal injury, with lower adverse reaction rate and higher safety, which is worthy of clinical promotion.
