Clinical Effect Analysis of Ultrasonic Intervention Combined with Dachaihu Decoction in the Treatment of Gallbladder Perforation

Abstract:
Objective To analyze and summarize the ultrasound image characteristics and interventional treatment of gallbladder perforation, and to observe the postoperative efficacy with the addition and subtraction of Dachaihu decoction. Methods The ultrasound image data and interventional treatment of 44 patients with gallbladder perforation admitted to Tianjin Nankai Hospital from January 2020 to December 2021 were retrospectively analyzed. Results All 44 patients underwent ultrasound examination and ultrasound intervention, and 17 cases (38%) of liver abscess formed by liver bed perforation, 27 cases (62%) of free surface perforation, including 19 cases of free effusion around the gallbladder and 8 cases of encapsulated effusion, and 44 patients underwent ultrasound-guided percutaneous liver puncture cholecystic catheterization and drainage (UG-PTGD) and liver abscess, abdominal puncture catheter drainage or aspiration, all of which drained purulent turbid bile, which confirmed that the gallbladder perforation. After surgery, Dachaihu decoction was given oral treatment, and the symptoms were significantly reduced within 72 hours. Conclusion Gallbladder wall defects, free or encapsulated fluid around the gallbladder, and liver abscess around the gallbladder are characteristic ultrasound findings of gallbladder perforation. Ultrasound intervention combined with traditional Chinese medicine therapy can play an important role in the diagnosis and treatment of early gallbladder perforation.
