健脾益肾方、生物反馈联合盆底肌功能训练治疗老年女性压力性尿失禁的效果观察

摘要:
目的观察健脾益肾方、生物反馈联合盆底肌功能训练治疗老年女性压力性尿失禁的效果。方法:将126例老年女性压力性尿失禁患者随机分为对照组和观察组,每组各63例。对照组给予盆底肌功能训练,观察组在对照组的基础上给予健脾益肾方联合盆底生物反馈内外合治,对比两组的尿流动力学相关参数、1 h尿失禁、24 h尿失禁次数、国际尿失禁咨询问卷简表(ICI-Q-SF)评分、盆底肌电位值、尿失禁生活质量量表(I-QOL)评分,监测血清中转化生长因子-β1(TGF-β1)、25羟维生素D(25-OHD)含量。结果:观察组患者的有效率显著高于对照组(P<0.05);与对照组比较,观察组治疗后的尿流动力学相关参数ALPP、MUCP、BC、QMax、PMU、VMCC均显著升高(P<0.05);1 h尿失禁量显著减少,24 h尿失禁次数显著下降(P<0.05);治疗4周、8周后,观察组患者的ICI-Q-SF评分均显著低于对照组(P<0.05);观察组治疗后的盆底肌电位值(65.32±7.19)μV显著高于对照组的(54.25±5.36)μV(P<0.05);治疗4周、8周后,观察组患者的I-QOL评分显著高于对照组,差异有统计学意义(P<0.05);与对照组相比,观察组治疗后的血清中盆底肌成纤维细胞增殖相关指标TGF-β1、25-OHD等含量均显著升高(P<0.05)。结论:健脾益肾方、生物反馈联合盆底肌功能训练利于减轻老年女性压力性尿失禁患者的尿失禁症状,提高生活质量,改善尿流动力学参数,提高血清中TGF-β1、25-OHD含量以增强盆底肌力,值得推广。

Effect of Invigorating Spleen and Nourishing Kidney Prescription, Biofeedback Combined with Pelvic Floor Muscle Function Training on Stress Urinary Incontinence in Elderly Women

Abstract:
Objective To observe the effect of invigorating spleen and nourishing kidney prescription, biofeedback combined with pelvic floor muscle function training in the treatment of stress urinary incontinence in elderly women. Methods 126 elderly female patients with stress urinary incontinence were randomly divided into control group and observation group. The control group was given pelvic floor muscle function training, and the observation group was given spleen and kidney strengthening prescription combined with pelvic floor biofeedback internal and external treatment on the basis of the control group. Urodynamics related parameters, 1h urine leakage, 24h urine leakage frequency, ICI-Q-SF score, pelvic floor muscle potential and urinary incontinence quality of life scale (I-QOL) score were compared between the two groups. The levels of transforming growth factor-β1 (TGF-β1) and 25 hydroxyvitamin D (25-OHD) in serum were monitored. Results Compared with the control group (70.00%), the effective rate of the observation group (95.00%) was significantly increased (P<0.05). Compared with the control group, the parameters related to urodynamics in the observation group after treatment were significantly increased (t=6.174,-5 . 317,-5.311,-6.801,-2.835,-2.515,P< 0 . 05 ). The amount of urinary incontinence at 1h was significantly decreased, and the frequency of urinary incontinence at 24 h was significantly decreased (t=14.042, 7.287, P<0.05). After 4 and 8 weeks of treatment, the ICI-Q-SF scores in the observation group were signi.cantly lower than those in the control group (t=7.830, 28.852, P<0.05). After treatment, the pelvic floor muscle potential (65.32±7.19) μV of the observation group was significantly higher than that of the control group (54.25±5.36) μV (t=9.401, P<0.05). After 4 and 8 weeks of treatment, the I-QOL score of the observation group was signi.cantly higher than the control group, which the difference was statistically signi.cant (t=3.999, 7.033, P<0.05). Compared with the control group, the contents of TGF-β1 and 25-OHD related to the proliferation of pelvic floor myofibroblasts in serum of the observation group were significantly increased after treatment (t=-5.360, -5.187, P<0.05). Conclusion The spleen and kidney strengthening prescription, biofeedback combined with pelvic .oor muscle function training can reduce the urinary incontinence symptoms of elderly female patients with stress urinary incontinence, improve the quality of life, improve the parameters of urodynamics, increase the serum TGF-β1, 25-OHD content to enhance pelvic .oor muscle strength, which is worthy of promotion.
