Combined Dotted Thread-ligation and Marsupialization of Wound Edges for High Anal Fistula

Abstract:
Objective To compare the clinical effect of treating high anal fistula with combination of dotted thread-ligating therapy with fistula tract stripped and marsupialization of wound edges of internal opening and traditional solid thread-ligating therapy. Methods 70 patients with high anal fistula were randomly divided into observation group and control group, which 35 cases in each.Patients in observation group were treatedwith combination of dotted thread-ligatingtherapy with fistula tract stripped and marsupialization of wound edges of internal opening , and patients in control group were treated with traditional solid thread-ligating therapy. The wound healing, postoperative pain, anal function score and recurrence within 12 months were observed after operation. Results The VAS scores of the observation group were (1.54±0.85), (1.14±0.77) and (0.88±0.53) on the 1st, 3rd and 7th day after operation, which were lower than those of the control group (2.71±1.10), (2.89±1.21) and (2.23±1.39). The wound healing time of the observation group was (28.10±4.19) days, which was lower than that of the control group (35.26±4.88) days.The Wexner score of anal function after wound healing in the observation group was (0.58±0.38), which was lower than that of the control group (2.39±1.15). In terms of VAS scoreof postoperative pain, wound healing time and Wexner score, the observation group was better than that of the control group (P<0.05). Conclusion The therapy of the observation group can better protect the rectal ring and the function of anal sphincter, relieve the postoperative pain and shorten the healing time.
