Clinical Study of Relationship between Urinary Matrix Metalloproteinase-2, Tissue inhibitor of Metalloproteinase-2 and Decreased Bladder Compliance by Obstruction

Abstract:
Objective To investigate the application value of matrix metalloproteinase-2 (MMP-2 ) and tissue inhibitor of metalloproteinase-2 (TIMP-2) in patients with bladder compliance changes caused by obstruction. Methods The clinical data of patients with bladder compliance changes caused by BPH from November 2016 to January 2020 were included in the study group, excluding neurogenic factors, urinary system tumor, diabetes, interstitial cystitis and other factors. Urodynamic examination of the enrolled patients indicated bladder outlet obstruction and 20 ≤ bladder compliance < 40 mL/cmH2 O. The control group was selected from patients with BPH caused by obstruction and BC ≥40 mL /cmH2 O. At least 20mL urine was collected and frozen at ―80 ℃for detection. Serum creatinine and cystatin were measured in each patient. The contents of MMP-2 and TIMP-2 in urine were determined by ELISA kit. Results 37 patients were included in the study group. There were no significant difference in the age, IPSS score, body mass index and overactivity bladder symptom score between the two groups. Except for bladder compliance, there were no significant difference in urodynamic parameters (residual urine volume, maximum urinary force, bladder initial sensory capacity) between the two groups (P > 0.05). The levels of MMP-2 and TIMP-2 in urine of the study group were (0.75±0.26) ng/mL, (359.8±123.7) pg/ mL, which were significantly higher than those of the control group (0.52±0.21) ng/mL and ( 289 . 8 ±101 . 9 ) pg/mL (P <0.05). Conclusion The expression of MMP-2 and TIMP2 in urine is closely related to the changes of bladder compliance caused by obstructive factors.
