防旋转股骨近端髓内钉联合术后中药外敷治疗股骨转子间骨折的临床效果研究

摘要:

目的:研究防旋转股骨近端髓内钉( PFNA)联合术后中药外敷对股骨转子间骨折( IF)的治疗效果及预后的影响。方法:选取 2016年 1月—2018年 7月我院收治的 IF患者 82例。随机等分成试验组及对照组。对照组予以动力髋螺钉治疗，试验组则予以 PFNA联合术后中药外敷治疗。比较两组临床疗效，骨折愈合时间、住院时间、医疗费用，以及治疗前后血清神经生长因子( NGF)水平，并比较治疗前后疼痛程度。结果:试验组 Harris髋关节功能评价优良率为 80.49%，高于对照组的 56.10%(P＜ 0.05)。试验组骨折愈合时间 [(14.25±2.34)周 ]、住院时间 [(20.24±3.55) d]以及医疗费用 [(2.15±0.53)万元 ]均低于对照组的 [(22.42±3.15)周 ]、[(29.43±3.83) d]、[(3.32±0.72)万元 ]，差异有统计学意义(均 P＜ 0.05)。治疗后试验组与对照组的血清 NGF水平高于治疗前，且试验组 [(957.43±112.25) ng/L]高于对照组 [(762.29±87.55) ng/L]，差异有统计学意义(P＜ 0.05)。治疗后两组 VAS评分均低于治疗前，且试验组 [(0.13±0.02)分 ]低于对照组 [(0.57±0.23)分 ]，差异有统计学意义( P＜ 0.05)。结论: PFNA联合术后中药外敷应用于 IF患者的疗效显著，可促进其早日康复。同时有利于改善血清 NGF水平，缓解疼痛，值得临床推广应用。
