紫草油联合美洲大蠊粉在低位肛周脓肿术后的应用

摘要:
目的:观察紫草油纱条联合美洲大蠊粉对低位肛周脓肿术后切口愈合的作用。方法:将 116 例低位肛周脓肿患者随机分为观察组和对照组各 58 例,均采用相同的术后抗感染及支持治疗。观察组采用美洲大蠊粉联合紫草油纱条换药,对照组单纯采用紫草油纱条换药。分别观察记录两组术后第 1、6、11、16 d 创面疼痛 VAS 评分、分泌物评分、肉芽生长评分、并发肛周湿疹例数、创面愈合时间及创面愈合率等指标。结果:观察组术后 1、6、11、16 d 疼痛 VAS 评分分别为 6.49±0.55、4.98±0.26、2.59±0.34、1.06±0.23,对照组为 6.53±0.36、5.96±0.42、4.66±0.40、3.19±0.31,观察组治疗后 6、11、16 d 疼痛VAS 评分均明显低于对照组(P<0.05);观察组术后1、6、11、16 d 创面分泌物评分分别为2.43±0.36、1.89±0.41、1.64±0.26、1.45±0.23,对照组为 2.13±0.53、1.66±0.29、1.52±0.49、1.36±0.19,观察组患者治疗后 6、11、16 d创面分泌物评分均明显低于对照组(P<0.05);观察组术后 1、6、11、16 d 肉芽生长积分分别为 2.51±0.53、1.59±0.49、1.01±0.39、0.71±0.28,对照组为 1.61±0.64、1.03±0.36、0.86±0.26、0.42±0.21,观察组术后第 11、16 d 肉芽生长积分明显高于对照组(P<0.05);观察组术后 6、11、16 d 创面愈合率分别为 29.35±2.12、46.53±3.18、93.62±4.96,对照组为 21.51±1.83、33.19±2.24、76.25±4.03,观察组术后 6、11、16 d 创面愈合率均高于对照组(P<0.05);观察组并发肛周湿疹例数明显少于对照组(P<0.05);观察组创面愈合时间明显少于对照组(P<0.05)。结论:紫草油纱条联合美洲大蠊粉换药,可显著减轻低位肛周脓肿术后创面疼痛,减少分泌物渗出,促进肉芽生长,缩短创面愈合时间。

Application of Comfrey Oil Combined with American Cockroach Powder in Postoperative Low Perianal Abscess

Abstract:
Objective To observe the clinical effect of comfrey oil gauze combined with American cockroach powder on postoperative incision healing of low perianal abscess, and to explore a kind of incision healing regimen that can be used to promote low postoperative perianal abscess. Methods A total of 116 patients with low-grade perianal abscess were randomly divided into observation group and control group. Both groups of patients used the same postoperative anti-infective and supportive treatment. The observation group used the American cockroach powder combined with comfrey oil gauze to change medicine, while the control group simply used comfrey oil gauze to change medicine. The VAS score, secretion score, granulation growth score, number of concurrent perianal eczema, wound healing time and wound healing rate of the wounds on day 1, 6, 11 and 16 after operation were recorded. Results The pain VAS scores of the observation group were 6.49±0.55, 4.98±0.26, 2.59±0.34 and 1.06±0.23 on day 1, 6, 11 and 16, respectively, and those of the control group were 6.53±0.36, 5.96±0.42, 4.66±0.40 and 3.19±0.31. The VAS scores of the observation group were significantly lower than those of the control group on day 6, 11 and 16 after treatment (P < 0. 05). The wound secretion scores of the observation group on day 1, 6, 11 and 16 were 2.43±0.36, 1.89±0.41, 1.64±0.26 and 1.45±0.23, respectively and those in control group were 2.13±0.53, 1.66±0.29, 1.52±0.49 and 1.36±0.19. The scores of wounds in the observation group were significantly lower than those in the control group on day 6 , 11 and 16 after treatment (P < 0 . 05 ); the granulation growth scores of the observation group on day 1, 6, 11 and 16 were 2.51±0.53, 1.59±0.49,1.01 ± 0 . 39 and 0 . 71 ± 0 . 28 , respectively, and those in the control group were 1 . 61± 0.64, 1.03 ± 0.36, 0.86±0.26 and 0.42±0.21. The granulation growth scores of the observation group were signi?cantly higher than those of the control group on day 11 and 16 after operation (P < 0 . 05 ). The wound healing rates of the observation group were 29.35±2.12, 46.53±3.18 and 93.62±4.96 on day 6, 11 and 16 after operation, and those in the control group were 21.51±1.83, 33.19±2.24 and 76.25±4.03. The wound healing rates of the observation group were higher than those of the control group on day 6, 11 and 16 after operation (P<0.05); the number of cases of perianal eczema in the observation group was signi?cantly less than that in the control group (P<0.05); the wound healing time in the observation group was signi?cantly less than that in the control group (P<0.05). Conclusion Comfrey oil gauze combined with American cockroach powder can significantly reduce postoperative wound pain, reduce secretion exudation, promote granulation growth, shorten wound healing time, and is suitable for clinical promotion.
